INSTRUCTIONS

1 - Select golf course from the drop down menu at the top of the page.

2 - Upload up to three pictures using your picture app or take pictures with your camera.

3 - Select from the drop down menus the green or fairway where the issue is located.

4 - If necessary add any additional information.

5 - Submit when ready.
